Job Title: Disability Support Worker

Job Type: Casual

Location: Various clients site(s)

Main Responsibilities:

  • Support participant to work towards achieving their individual goals.
  • Support in day to day living, cooking, housekeeping, hygiene, Medications, community access etc.
  • Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.
  • Report regularly to manager
  • Monitor and review participant’s progress through the service and against the support plan.
  • Progress note writing and clients record keeping
  • Support clients to participate in activities and tasks including support and community access, transport to appointment and medicals.
  • Support with the administration of medication and adhering the medication and administration policy.
  • Monitor the safety and well-being of the clients and report any concerns to the Manager.
  • Work with clients, family/ carers, relevant case managers and other professionals to support the client’s individual goals.
  • Management and support client’s goals towards addressing behavioural issues and goals of care
  • Ensure that all programming undertaken is appropriately maintained, documented, and reviewed.
  • Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
  • Liaise and collaborate with clients, their community and community support agencies.
  • Actively contribute to a team environment through open communication, participation in regular staff meetings, planning processes.
  • Provide back up and support to other staff, as necessary.
  • Carry out roles and responsibilities as detailed in the relevant policies and processes.

Qualifications and Experience

Cert III Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar. Minimum 6 months experience preferred.
